What to Expect

Like good health and youth, most of us take our locks for granted — that is, until they’re gone. For many people, a hair transplant can help bring back what looks like a full — or at least a fuller — head of hair.

If thinning up top or going bald really bothers you, the procedure can be one way to feel more confident about your looks. But first, talk with your doctor about what you can expect during and after the surgery.

What Is a Hair Transplant?

It’s a type of surgery that moves hair you already have to fill an area with thin or no hair. Doctors have been doing these transplants in the U.S. since the 1950s, but techniques have changed a lot in recent years.

You usually have the procedure in the doctor’s office. First, the surgeon cleans your scalp and injects medicine to numb the back of your head. Your doctor will choose one of two methods for the transplant: follicular unit strip surgery (FUSS) or follicular unit extraction (FUE).

With FUSS, the surgeon removes a 6- to 10-inch strip of skin from the back of your head. He sets it aside and sews the scalp closed. This area is immediately hidden by the hair around it.

Next, the surgeon’s team divides the strip of removed scalp into 500 to 2,000 tiny grafts, each with an individual hair or just a few hairs. The number and type of graft you get depends on your hair type, quality, color and the size of the area where you’re getting the transplant.

If you’re getting the FUE procedure, the surgeon’s team will shave the back of your scalp. Then, the doctor will remove hair follicles one by one from there. The area heals with small dots, which your existing hair will cover.

After that point, both procedures are the same. After he prepares the grafts, the surgeon cleans and numbs the area where the hair will go, creates holes or slits with a scalpel or needle, and delicately places each graft in one of the holes. He’ll probably get help from other team members to plant the grafts, too.

Depending on the size of the transplant you’re getting, the process will take about 4 to 8 hours. You might need another procedure later on if you continue to lose hair or decide you want thicker hair.

Expectations and Recovery

After the surgery, your scalp may be very tender. You may need to take pain medications for several days. Your surgeon will have you wear bandages over your scalp for at least a day or two. He may also prescribe an antibiotic or an anti-inflammatory drug for you to take for several days. Most people are able to return to work 2 to 5 days after the operation.

Within 2 to 3 weeks after surgery, the transplanted hair will fall out, but you should start to notice new growth within a few months. Most people will see 60% of new hair growth after 6 to 9 months. Some surgeons prescribe the hair-growing drug minoxidil (Rogaine) to improve hair growth after transplantation, but it’s not clear how well it works.



Risks and Costs of Treatment

The price of a hair transplant will depend largely on the amount of hair you’re moving. Most insurance plans don’t cover it.

As with any kind of surgery, transplants have some risks, including bleeding and infection. There’s also the chance for scarring and unnatural-looking new hair growth.

Around the time new locks start to grow, some people have inflammation or an infection of the hair follicles, called folliculitis. Antibiotics and compresses can relieve the problem. It’s also possible to suddenly lose some of the original hair in the area where you got the new strands, called shock loss. But most of the time, it’s not permanent.

It is the conventional method of hair transplant specifically for someone above grade 4 as per Norwood’s classification in which a stripe is harvested from the permanent donor area on the back and the side of the scalp. The harvested grafts are closed with sutures and can be removed after primary epithelization of skin which is usually 2 weeks. Instructions Before & After Hair Transplant:-
Inform your doctor of any known allergies, previous surgeries and any other medications you are following.
Inform the doctor if you had an event during the administration of local anesthesia or you have never had local anesthesia before.
Blood pressure, diabetes, epilepsy, thyroid, medicines, can be continued. Take morning dose on the day of the procedure. Aspirin and anticoagulants can be continued after informing the doctor.
Stop, multivitamins Minoxidil, Mintop, Tugain, Rogain & hair growth medicines 5 days before the procedure.
Stop Smoking one week prior and three weeks after the procedure.
Do not wear T-shirts, pullovers, on the day of the hair transplant.
Have light breakfast 30-40 minutes prior & reach the hospital by 8.15am on the day of the procedure.
Keep 3-5 inch long hair on the back to cover the shaving of the donor area.
Shaving of the head as required for FUE will be done on the morning of the procedure.
Shaving for body hair can be done 5 days prior and for beard hair grafts, keep the unshaven beard for 2 days prior.

Medicines to be taken for 5 days after the Hair transplant

From day 6 to day 10 there are no medicines. Start minoxidil & hair growth vitamins from day 10.

One dose of the medicines have been administered before starting the procedure. Take your evening dose when you reach home.



Care to be taken after Hair Transplant

You need 2 days of rest at home. You can work in a dust free AC area.
You can wear a simple cap loosened from the back to cover the grafts immediately after the transplant. Do not adjust or move the cap while wearing, the cap should not rub against the grafts or give pressure on the grafts.
Use spray over the grafts every 3-4 hours round the clock for first 3 days to prevent dryness and itching.
Cut your nails short after the hair transplant to prevent accidentally scratching the grafts or the donor area.
Use ice cold pack on the forehead followed by firm finger massage against the forehead from mid-forehead to the sides, for 10 minutes 3 times a day. This will avoid swelling. The cold pack can also reduce itching and pain.
Remember to have your evening dose of medicines, additional painkiller tablet can be taken at night if required.
Sleep on your back. Do not sleep on side or face. You can sleep propped up with extra pillows to avoid swelling.
You have a dressing on back of the head, remove it after 2 days. If you remove it immediately, it may leave blood stains on the pillow. Do not panic. Report to the clinic for large stains or rapidly increasing blood stains.
No head bath for 2 days. Lukewarm shower on 3rd Wet the hair spread medicated shampoo lightly with fingers flat and have medium pressure shower. Do not rub on the grafts till 10 days. Shampoo alternate day. Dry the hair gently by pressing with a double folded towel.
Some medicines advised after the transplant, are for 2 days and others for 5 days. From day 6 to day 10 there is no medicine. After 10 days start all the hair growth medicine and continue regular hair care program.
After 10 days it is essential to run with the ball of your fingers on the grafts and remove loose scabs over next few days. Do not hold back rubbing the area thinking it will hurt the grafts, now grafts are healed and need to be cleaned. Oil or Vaseline used overnight can soften the scabs and make them easy to remove.
Small 2 mm hair seen in the grafts may fall off with the scabs, do not worry, these will grow back after 4 months.
No hair growth is seen up to 4 months. After 4 months 1-2 mm hair start growing from the grafts and then continue to grow 1 – 1.5 cm every month. More new hair growth occurs up till 9-10 months after the procedure.
Rarely you may accidentally scratch or pull out a graft causing bleeding from the site. Apply firm pressure for 2 minutes and the bleeding will stop. Report to the clinic in any such event.
Itching can be caused by dryness. Apply Vaseline body lotion, Ultiderm lotion or Lobate GM cream to reduce itching. OR have antihistamine tablet Allegra 120 twice a day for 3 – 5 days to reduce itching.

Dense Hair Transplant.
One hundred follicular units transplanted into 1cm2 can achieve a survival rate greater than 90%.
The effectiveness and naturalness of the results of follicular unit (FU) transplantation is well proven, but the problem of hair growth yield with dense packing is a concern for many surgeons. How densely can the grafts be packed without harming their survivability? This issue has become the object of debate in recent years, especially in the era of mega sessions when lots of clinics around the world offer the possibility of transplanting large quantities (3,000-6,000) of FUs in one surgery. Six years ago, when we attempted to increase the density of transplanted grafts with the purpose of achieving more natural results, the safe density for the survival of grafts was considered to be 25 FUs per cm². The research of Mayer, et al. indicated that the implantation of more than 30 FUs in 1cm² might reduce hair growth by 20-30%.4
It was postulated by others that the insertion of 40 or more FUs into 1cm² could reduce graft survival because of compression.3,4,7 What is the reason for the reduction of graft survival, if it really occurs, in the circumstance of dense packing?

As a result of analyzing this problem, we can list four possible factors:

“Lateral pressure” on the implanted grafts in very small recipient sites
“Ultra fine” preparation of grafts, leading to the absence of tissue around follicles, or artificial splitting of FUs with the purpose of enabling their implantation in smaller sites
Crush injury of grafts when assistants push them into small recipient sites
Violation of blood circulation in the recipient site by excessive incision density
Mechanically, high density cannot be achieved without reducing the size of recipient sites. In our opinion, implantation in small recipient sites, the sides of which tightly press grafts and provide maximum contact of the implant with the surrounding tissues, is optimal for survival. In this way, oxygenation is restored faster, which is the main factor enhancing the survival of grafts. At the same time, small sites reduce bleeding and loss of grafts.2,3 Recipient sites ranging from 0.8mm to 1.25mm have long been considered ideal for grafts containing 1-4 hairs. Accurate preparation under stereoscopic microscopes makes it possible to achieve intact FUs, which are coated with fine layers of the dermis but don’t contain extra tissue and epidermis. Such FUs have high survival capability and are easy to implant in recipient sites of <1.5 mm.2,8

Although graft insertion in closely located small recipient sites is difficult for many assistants, especially at the beginning, we have come to the conclusion that this is not a technological problem but purely an issue of training. Our experienced assistants can fill 10-12 recipient sites of ≤1mm per minute. Moreover, they would prefer to work on small recipient sites because they cause less bleeding and graft popping.
